Abstract
This research paper focus on the right drugs is a crucial part of treating chronic conditions. Hypertension, is chemic heart disorders, diabetes mellitus, and dyslipidemias are significant health issues of the 21st century and incur higher costs. In developing nations, 79% of projected fatalities are caused by chronic illnesses. Not taking prescribed drugs is one of the biggest health risks in the globe. Patients with chronic conditions often see different healthcare practitioners, visit numerous pharmacies, and consume various medications, leading to medication non-adherence. The new primary care models indicate a lack of encouragement for strong interactions among physicians, patients, and pharmacists. Training for the management of chronic illnesses is seldom appropriate, and there is little consistency and communication among healthcare practitioners. Community pharmacists are the best people in the health care field to help manage patients with chronic diseases. To help people take their medications as prescribed, numerous community pharmacies provide Synchronization services. These programs function by getting over the problems that make people not take their medicine by renewing their prescriptions during their one visit. There are several medication synchronization programs accessible in community pharmacies, however the effectiveness of these programs depends on the sort of synchronization program that is used.
